NRSA Progress Reports

Please note: The paper 416-9 progress reports will soon be replaced by electronic RPPR reports, which will be completed in eRA Commons. This change will take place no later than November 2012, but it may be implemented at any time. Before beginning the paper 416-9 form described below, please log in to eRA Commons to see if the eSNAP tab has been replaced with the RPPR tab. NRSA Fellows must submit a progress report detailing their activities every year.

  • Please note that the sample form does not include all necessary information.  For example, you will need to fill in your training site address and telephone information.  Also, boxes 7b and 8b only need to be filled in if you have human or vertebrate animal subjects, respectively.
  • In section 2d, the Major Subdivision will be General Campus or School of Medicine or Skaggs School of Pharmacy and Pharmaceutical Sciences.
  • If you have human subjects or are using vertebrate animals, be sure to include your most recent IRB or IACUC approval date.  The agency will not accept the report if you list an approval date that has now expired.
  • In section 17, the answer to the “Supplementation of Stipend” should usually be “No.”  If you think your stipend has been supplemented, please double-check with your fund manager to verify that this is true and to determine the source of the supplemental funds (e.g., campus discretionary funding).  If you have received supplemental funds, indicate the fund source, the dates for which you received the supplement, and the supplemental amount.

Once the fellow’s sections are complete, send the form to the sponsor, who will complete section 18.

Once the sponsor completes section 18, submit the entire report to the Office of Graduate Studies at least 14 days prior to the due date.  For example, if the report is due to the NIH on November 1, submit the form to OGS no later than October 18.

Dean Barrett will review and sign the report (assuming there are no problems).  OGS will contact the student when the signed report is ready for pickup.

It is the student’s responsibility to submit the progress report to the NIH agency.  Some agencies allow submission by fax, while others require submission by mail.  If you are unsure of how to submit your progress report, contact your grants management specialist.
